I believe my router is defective. Below are the results of troubleshooting while connected to a cable modem and SSID names configured to be the same for 2GHz and 5GHz. All wifi connections are from an iPhone. Also tried with different SSIDs for 2GHz and 5GHz and still no connection to 5GHz.

2GHz disabled and 5GHz enabled, no wifi.
Tried manually connecting to SSID but network not found.

2GHz enabled and 5GHz disabled, connection to wifi is successful.
Speedtest is about 80Mb download and 10Mb upload.

2GHz enabled and 5GHz enabled, connection to wifi is successful.
Speedtest is about 18Mb download and 5Mb upload.

Speedtest on wired connection to laptop is 485Mb download and 11Mb download.

Power cycled modem, disconnected first router, connected an identical pcWRT router with the same firmware to the cable modem with SSID names configured to be the same for 2GHz and 5GHz.

2GHz enabled and 5GHz enabled, connection to wifi is successful.
Speedtest is about 478Mb download and 11Mb upload.

Also connected to SSID configured for 2GHz only.
Speedtest is about 109Mb download and 10Mb upload.

Speedtest on wired connection is 488Mb download and 11Mb.
